Proof of Residency (tax or utility bill)

Recent Photo (Head shot) of your child.

Original Birth certificate and 1 copy.

We will be accepting Cash, Check, Money Order, *Visa/MC Credit/Debit Cards. * There is a $5 processing fee associated with using Visa/MC Credit/Debit Card.

Mothers/Guardians Month & Day of birth. (for National Roster Program)

*Registration forms will be available on our website and at registration location.

Coach/Student Demo Info

For anyone who is considering being a coach or student demo (under the age of 18), you must fill out a volunteer application form and hand it in during registrations.

Potential coaches must also bring with them a copy of their driver's licensce and either their original or copy of their social security card.

If ALL of the required documentation is not correctly filled out and handed in during registrations, your coaching application WILL NOT be accepted!
