                                         Physical form - Due at Handouts on July 26 (Physicals will be given at the field for $15 on Wednesday, July 16)
                                        Must be completed by Physician, dated Jan. 1, 2008 or later and requires physician's official stamp. These are National rules and no
                                        exceptions can be made!

                                          Injury form - Fill out Section B, hand into League Rep (J.Crosio) to fill in Section A and turn in for payment.
                                        **Must be turned in as soon as possible to avoid processing delays.

                                          Parental consent form - Must completed and handed in at uniform handouts on July 26
